Calvert Internal Medicine Group is seeking a full time Certified Medical Assistant. Must have experience in performing vitals, venipunctures, and injections. Superior organizational and communication skills essential. MEDENT experience preferred but not required. Candidate must be able to travel to multiple offices. Care Manager, RN – Dunkirk

Calvert Internal Medicine Group is seeking an ambulatory care manager. The care manager will collaborate with practice providers to identify patients needing care coordination and develop individual care plans with patient-specific goals and interventions. The duties include performing comprehensive patient assessments, coordinating referrals, patient education, clinical documentation, reviewing clinical data and metrics, and serving as a patient liaison and advocate for local hospitals/health systems, nursing centers, home health, and community providers.

The position requires an active RN licensure with the Maryland Board of Nursing. Candidates must have at least three years of clinical experience and possess strong clinical assessment and chronic disease management skills. Knowledge of population health and value-based programs is preferred. CIMG offers a competitive annualized salary range from $72,000 to $100,000, depending on qualifications and expertise.
