After completing the registration form, our team will review your information and contact you to schedule your first appointment or gather any additional details if necessary.

Basic details such as your full name, date of birth, sex, and contact information (phone number and email address). Don’t have an email address? No problem! There is an option to indicate that when registering.

If you have insurance, please have the following details ready:

  • Insurance Name
  • Plan Name
  • Subscriber ID
  • Group Number/Plan Code
  • If you do not have insurance, simply check the box that says "I do not have insurance" on the form.

You’ll be asked for your full street address, city, state, and zip code to ensure that we have up-to-date information for our records.

To help us provide the best possible care, we ask you to share your race, ethnicity, and preferred language. Not comfortable providing this information? You can select “Decline to Specify” for each question.

You will also have an opportunity to share the reason for your appointment so that we can better prepare for your visit.

How do I schedule an appointment?

How will I receive appointment reminders?

If you have consented to reminders, you'll receive:

  • Email: 5 days before the appointment
  • Phone Call: 3 days before the appointment.
  • Text Message: 2 days as well as 2 hours before the appointment.

*Please note, all notifications will display your expected arrival time, which is 15 minutes prior to your scheduled appointment time.*

What happens if I need to cancel or reschedule?

You must provide at least 24 hours notice to avoid a no-show fee.

Why does my appointment reminder say a different time than what I booked?

Your reminder includes an arrival time set 15 minutes earlier than your scheduled appointment. This helps ensure we have enough time to check you in and complete any necessary paperwork before your appointment starts. For example, if your appointment is scheduled for 8:00 AM, your reminder will say 7:45 AM as your arrival time.

Do I need to arrive even earlier than my reminder time?

No, you don't need to arrive earlier than the reminder time. We've already factored in the extra time needed before your appointment. Arriving at the time shown in your reminder ensures a smooth process.

What if I arrive right at my scheduled appointment time?

While we can still see you, arriving at the reminder's arrival time helps us stay on schedule and reduces waiting times for everyone. We appreciate your help in arriving at the time indicated in your reminder.

Can I check in before my appointment?

Yes, you can use Pre-Check-in, which sends a link 2 days and 2 hours before your appointment. Complete parts of the check-in process from your phone or tablet.

What should I bring to my first appointment?

Please bring your insurance card, photo ID, and a list of current medications.

Do I need to bring any medical records with me?

Yes, if you have them, otherwise we request that you complete a medical records release form. This form allows us to obtain healthcare information from your previous providers, ensuring comprehensive care.

If you are not provided with this form upon arrival, please ask our receptionist for one.

Does the office offer Telehealth services?

Yes, after the initial visit, you can schedule a Telehealth appointment if you prefer to see a provider without coming into the office. Call 410-535-2005 to arrange a Telehealth visit.

How do I request a medication refill?

Contact your local pharmacy first. If no refills are available, use the Patient Portal to request electronically. Controlled medication refills require an appointment.

What is your privacy policy?

Your health information is protected under HIPAA. If you'd like to authorize someone to access your medical records, you'll need to complete a form in person.

What are my rights as a patient?

You have rights to inspect, amend, and receive copies of your medical records. For a detailed list of your rights, visit this link.

What is your controlled substance prescribing policy?

Our controlled substance prescribing policy includes the following important guidelines:

  • Medication Management: You must inform us of all medications you're taking and follow your prescription exactly as directed
  • Appointment Requirements: Routine visits are required at least every 90 days, and you must be seen in the office at least once every six months.
  • Testing and Safety: You must cooperate with any urine tests, pill counts, and keep your medications in a secure place. Please note, lost prescriptions will not be replaced.
  • Prescription Refills: Refills are provided only during regular, scheduled appointments. No refills are available on nights, weekends, or holidays.

These guidelines help us ensure safe and effective care for all patients.

How can I communicate with my healthcare provider?

The best method is through the Patient Portal, where you can send non-urgent messages to your provider.
